The first step to becoming an Innovation Pointe client is to submit the following forms and documents:
These forms can be filed electronically using Adobe Reader, which can be downloaded for free from adobe.com. You may either e-mail the completed forms to julie@evansvillegage.com or print and mail them to the following address:

Once we have received this information, we will contact you within a few business days to set up an appointment to discuss your needs and what Innovation Pointe can offer you. At that point, you will be asked to submit a formal application and all other required documents. For informational purposes, you may view the application and required documents.
